Liberty Healthcare conducts assessments and needs-based eligibility determinations for North Carolinians who are applying for Medicaid-funded services. These assessments help individuals receive appropriate levels of home and community-based services.

Due to anticipated organizational growth and an expansion of services, Liberty is now hiring for a newly-created full-time “Independent Assessor” position.

Your typical day as an Independent Assessor with will involve:

  • Driving throughout the region and meeting with clients in their homes
  • Administering a standardized assessment to each client
  • Assessing each client’s overall health status, diagnoses, and other pertinent health factors
  • Using a laptop to complete a comprehensive report which will be used to determine the client’s eligibility for various Medicaid-funded services
  • Providing consultation to clients / beneficiaries
  • Collaborating with various stakeholders to help individuals live as independently as possible in their home / community

Please note – there are no direct nursing patient care responsibilities associated with this position.

Skills/Requirements

You’re a good fit for this position if you possess these attributes:

  • Registered Nurse license in North Carolina
  • Person-centered thinking, planning and interviewing skills
  • Ability to effectively serve people of all ages who have a wide array of abilities and disabilities
  • Prior experience in home health, hospice, case management or a similar remote nursing role
  • Ability to use a laptop effectively
  • Willing to use your vehicle to travel throughout the region each day

Familiarity with some of the following areas is a plus:  CMS / NC Medicaid, Home and Community-Based Services (HCBS) waivers, Personal Care Services (PCS), Community Alternatives Program for Disabled Adults (CAP DA), and Community Alternatives Program for Children (CAP C).
