Job Description

 

The Food and Nutrition Department on the Sarah Neuman Campus  is seeking  a full time Registered Dietitian  to join our team! This position will sit in 

Mamaroneck , New York and will report from Monday to Friday, 9 am to 5 pm, no weekends.

Job Summary: Maintains all clinical nutritional services for residents in each community. Acts as liaison between Nursing and Food and Nutrition staff. The function of the position actively incorporates the Mission and Vision of the New Jewish Home.

Reports To: Director, Support Services

Duties and Responsibilities:

 Develops and implements care plans for residents based on nutritional needs in conjunction with other disciplines. Responsible for initial, annual and quarterly assessments on assigned residents within assigned units.

 Makes initial nutritional assessment of assigned residents; reassesses and revises assessment at least quarterly or more frequently if necessary.

 Attends and participates in the Interdisciplinary Care Plan Meetings.

 Evaluates nutritional care and provides follow-up as needed for continuity of care.

 Acts as the liaison among residents, nursing staff in addition to food and nutrition staff, in relation to nutritional services.

 Conducts meal rounds to review resident likes and dislikes.

 Assists in the development and presentation of in-services and nutrition education materials for facility staff as needed.

 Plans, calculates, and modifies normal and special diets as prescribed by physician’s orders.

 Monitors diet prescriptions with physician orders and modifies as needed.

 Is available for diet counseling.

 Assists the community in PI programs/audits.

 Assists in the development, implementation and revision of cycle menus for general and therapeutic diets.

 Maintains accurate and updated computer records on nursing home residents.

 Responds to residents served based on developmental age.

 Must understand and respect cultural and religious beliefs of residents served.

 Other duties as assigned.

 

 

 

Skills & Requirements

Requirements:

  • Registered Dietitian.
  • Two years clinical experience in long term care preferred.

Specialized Skills and Competencies:

 Demonstrates the ability to assess the nutritional status of patients and residents in Sub[1]Acute and Long-Term care setting.

 Completes documentation which meets best practice and regulatory guidelines.

 Develops measure goals within individualized care plan

 Participates in interdisciplinary care conferences.

 Demonstrates effective communication skills.

 Ability to communicate and respond appropriately to the needs of the geriatric population
