Job Description

The Memory Care Program Manager oversees the daily operations of the Memory Impaired (MI) program within the Adult Day Health Care Program. This role provides staff supervision, coordinates person‑centered care, supports caregivers, and ensures high‑quality, compliant program delivery.

Key Responsibilities

  • Oversee all aspects of the Memory Impaired program, including admissions, attendance, activities, staffing, and scheduling
  • Supervise, train, and support staff, interns, and volunteers
  • Complete and maintain social assessments, care plans, and required documentation
  • Lead care planning meetings and communicate plans with caregivers and community partners
  • Coordinate caregiver support groups and collaborate with external agencies
  • Partner with transportation staff to ensure safe and efficient services
  • Respond to registrant needs and emergencies as required
  • Maintain ongoing communication with program leadership

 

Skills & Requirements

Qualifications

  • Experience working with individuals with memory impairment and/or dementia
  • Supervisory experience in healthcare, social services, or adult day programs
  • Strong organizational, communication, and leadership skills
