Job Description

Registered Nurses are encouraged to consider an outstanding career opportunity with Liberty Healthcare Corporation.

Liberty Healthcare operates a program in the District of Columbia which serves people who are applying for publicly-funded long term care supports and services. The goal of this program is to enable people to receive the support services they need to live as independently as possible – while applying a valid, reliable and quality assessment process to prevent Medicaid fraud, waste and abuse. Specific services include intake, applicant eligibility assessment, level of care determinations, recommending a range of service hours, and linking beneficiaries to home health providers.

Liberty Healthcare has ongoing needs for full-time “Nurse Assessors” within the program. This is a field-based position which ordinarily requires regular daily travel to client homes throughout the DC Metro Area – however due to current public health concerns related to COVID-19 – Nurse Assessors will be temporarily performing client assessments remotely / telephonically from their home offices. Nurse Assessor in-person home visits will resume as soon as possible.

Your responsibility as a Nurse Assessor is to assess client health and environmental factors to help determine their eligibility for Medicaid-funded services. This role is limited to conducting nursing assessments and does not involve direct patient care.

Specific responsibilities will include assessing client’s activities of daily living and evaluating client’s home environment – and completing electronic reports related to client’s health status, diagnoses, medications and other pertinent factors.

As a Nurse Assessor and full-time employee, you can expect and annual salary of $88,000 and comprehensive benefits package.  When in-home assessments resume, you will also be regularly reimbursed for approved travel expenses.
