Job Description  *This position has a sign-on bonus of $3,000 along with a $500 referral fee for other LPN/RNs you refer for the company* 

Hourly rate: $26-$32/hr 

Position Summary: 

The LPN plays a vital role in supporting the health and well-being of individuals with Intellectual and Developmental Disabilities (ID/DD), Severe Mental Illness (SMI), behavioral challenges, or dual diagnoses across multiple clinical facilities. This role involves providing direct medical care and ensuring compliance with all medical protocols and best practices. 

Key Responsibilities: 

  • Medical Oversight & Consumer Health: 

  • Monitor the medical condition of residents regularly and coordinate appropriate care under RN or physician guidance. 

  • Track and document vitals (blood pressure, pulse), weight, and other health metrics per program requirements. 

  • Maintain and audit medical records to ensure timely medical, dental, and psychiatric appointments. 

  • Respond to medical emergencies and provide necessary care or referral. 

  • Ensure infection control procedures are followed in all settings. 

  • Consult with RNs, physicians, and families regarding health issues, treatments, or changes in condition. 

  • Medication Management: 

  • Administer medications as prescribed and supervise MAC workers in proper medication administration procedures. 

  • Ensure compliance with medication protocols, including documentation, secure storage, and regular audits. 

  • Review medication logs daily for discrepancies or refill needs; update RN accordingly. 

  • Prepare Medication Administration Records (MARs) for new or transitioning residents. 

  • Provide training and oversight to direct care staff regarding medication policies and procedures. .

  • Consumer Care & Support: 

  • Support the physical and hygiene needs of residents as needed. 

  • Promote consumer rights, dignity, and independence. 

  • Assist in planning and implementation of health-related groups (e.g., nutrition, hygiene, wellness). 

  • Participate in team-based care planning and implementation of behavior or support plans. 

  • Documentation & Compliance: 

  • Complete all required documentation, including progress notes, incident reports, and daily logs, in a timely and accurate manner. 

  • Maintain compliance with all licensing, regulatory, and agency policies. 

 
Skills & Requirements

Qualifications:  

  • Licensed Practical Nurse (LPN) in good standing with the state of employment. 

  • Nursing degree from an accredited program.  

  • Experience with individuals with ID/DD, behavioral health needs, and medically fragile conditions preferred.  

  • PCM Practitioner or Level 2 (as required by site) certification; MAC supervision experience a plus.  

  • Strong clinical judgment, documentation, interpersonal, and organizational skills. Ability to work independently across multiple sites.  

  • Strong communication and interpersonal skills.  

  • Ability to work with compassion, patience, and flexibility in a variety of community settings.  

  • Ability to maintain confidentiality and adhere to professional boundaries.  

Transportation Requirements:  

  • Provide transportation for consumers as needed using agency or personal vehicles.  

  • Maintain a safe driving record and valid driver’s license with personal liability insurance.  

  • Follow all agency transportation policies, including seatbelt usage, safe cellphone practices, and vehicle documentation.  

  • Immediately report any vehicle issues, accidents, or violations to appropriate personnel.  

  • Must have reliable transportation, maintain a safe driving record and valid driver’s license with personal liability insurance.  

Work Environment & Schedule:  

  • This is a non-exempt, full-time position.  

  • This position requires in-person work and is not eligible for remote or hybrid work arrangements.  

  • Requires travel between multiple residential program sites.  

  • Fast-paced environment involving direct interaction with consumers and staff.  

In-Service Training & Continued Education Requirements:  

  • Participation in all agency-required in-service trainings.  

  • Commitment to ongoing professional development to enhance skills and stay informed on best practices in behavioral health.  

  • Completion of onboarding and any continuing education requirements as directed by the agency or supervisor.  

  • Attend staff meetings, trainings, and in-service sessions.  

  • Maintain licensure and participate in ongoing education related to mental health, ID/DD care, and medication administration.
