About the Position
This position is primarily responsible for the care of patients assigned by the Hospice Admissions RN. The RN assigned will do the admission assessment for that patient, identifying patient needs and status. The RN, under the physician’s nursing order, Will devise a Plan of Care to address all patient needs to provide optimal functioning.
This position is also eligible to receive an annual team performance bonus.
Job Duties
• Presents hospice services and philosophy of care to patients and families faced with the challenges of arranging end of life care.
• Performs in person assessments to determine eligibility for hospice, receives certification from the hospice physician and obtains consent for service.
• Completes admissions process, initiates plan of care and ensures immediate care needs are addressed.
• Communicates with the receiving interdisciplinary team about the goals of care, initial plan of care and patient/family expectations.
• Provides direct nursing care in a Hospice setting under the guidelines of a physician’s order.
• Performs/completes admission/start of care (SOC), recertification, evaluations, discharges and accompanying documentation in a timely manner.
• Performs a detailed assessment resulting in the development of a patient specific Plan of Care. Client clinical record documentation is done per agency policy.
• Communicates with other team members regarding the Plan of Care and continues to work with team members for the duration of the patient’s case.
• Effectively communicated with referring physician, either verbal or written, and provides accurate treatment notes and other necessary documentation.
• Documents information regarding interpretation of Hospice care policy, agency philosophy, objectives, and long-range plans.
• Prepares for patient discharge planning as soon as possible along with informing and preparing the patient, team members and physician.
• On-going observational recording/reporting of changes in patients condition as soon as possible to the patient’s physician, and other team members,
• Makes recommendations for involving other services, as deemed necessary.
• Advises and consults with the patient’s family in the patient’s therapy program.
• Attend staff meetings at least 4 times/year.
• Documents all communication with other personnel to contribute to patient’s case conference notes.
• Uses equipment and supplies effectively and efficiently and request supplies needed for patient care.
• Maintains confidentiality of patient information as well as co-worker’s personal information if applicable at all times.
• Implements infection control and safety measures as appropriate.
• Consistently addresses patients concerns. Promotes education for patients, family members and caregivers.
• Demonstrates current knowledge regarding professional career.
• Attends mandatory in-services and maintains 100% compliance with mandatory data-based training.
• Participates in orientation of Hospice/Home Health care staff members.
• Immediately reports abuse/suspected abuse to supervisor, and DHHS or APS.
Job Requirements
• Graduate of an accredited school of nursing.
• Licensed to work as a Registered Nurse in the Commonwealth of Virginia.
• At least on year acute care experience, Hospice preferred.
• Current CPR card.
• Ability to communicate effectively in English, both verbally and written.
• Knowledge of the duties and functions of Home Health and Hospice care and its relationship to the healthcare community is preferred.
• Must know or willingness to learn and perform HIS documentation and SOC.
• Must possess the ability to make independent decisions and work independently thought out the workday.
• Possess a driver’s license, proof of automobile insurance and readily available, reliable transportation.
• Must possess the ability to deal tactfully and compassionately with patients, family members and caregivers.
• Must be willing to and able to participate in a teamwork approach, working with all disciplines involved in the patient’s care/case.
