Salary: $80 - $90k
About the Position
The RN Clinical Liaison serves as the primary clinical representative for Home Health and Hospice services, developing referral relationships, coordinating admissions, performing clinical assessments, educating patients and families, and ensuring smooth transitions of care.
This position is primarily responsible for the clinical management of a caseload of patients facing end of life and working with the interdisciplinary team to create a plan of care that meets the patient/family needs and goals.
This position is also eligible to receive an annual team performance bonus.
Job Duties
- Develop and maintain relationships with hospitals, physicians, skilled nursing facilities, assisted living communities, case managers, and referral sources.
- Promote Home Health and Hospice services through outreach and education.
- Review referrals and assess clinical appropriateness and eligibility.
- Perform bedside and community-based assessments as needed.
- Educate patients and families regarding services and obtain required consents.
- Coordinate timely admissions with clinical and operations teams.
- Collaborate with physicians to obtain required orders and documentation.
- Ensure accurate, timely EMR documentation.
- Provide outstanding customer service and resolve referral barriers.
- Maintain compliance with Medicare, state regulations, accreditation standards, HIPAA, and agency policies.
Job Requirements
- Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
- Minimum two years of Home Health, Hospice, Hospital, Case Management, or Care Coordination experience.
- Excellent communication, assessment, organizational, and relationship-building skills.
- Valid driver's license and reliable transportation.
- Home Health/Hospice experience and EMR experience preferred.
