About the Position
The Hospice Psychosocial Clinical Manager leads psychosocial services that support the emotional, social, spiritual, and practical needs of patients and families. The manager supervises assigned social work, spiritual care, and bereavement staff; supports coordinated, individualized care; and promotes staff development and quality improvement. The role includes direct patient and family support, helping cover bereavement services, filling in for other clinical managers, and other duties as assigned.
Job Duties
Psychosocial leadership and patient care

Supervise assigned team members, coordinate schedules and caseloads, arrange coverage, and monitor visits and documentation to support timely services and manageable workloads.

Participate in hiring, orientation, competency assessment, performance evaluations, coaching, and corrective action. Arrange discipline-specific clinical supervision by appropriately qualified professionals.

Oversee psychosocial assessments and care plans addressing patient goals, caregiver stress, coping, family dynamics, financial concerns, cultural needs, and available support systems.

Provide social work visits and patient or family support during vacancies, absences, urgent needs, or increased census, within professional qualifications and scope of practice.

Participate in interdisciplinary group (IDG) meetings, lead family conferences as needed, and coordinate advanced care planning, community resources, care transitions, and placement support.

Partner with chaplains and other clinicians to address spiritual needs, family conflict, ethical concerns, and safety risks while respecting individual beliefs, preferences, and choices.
Bereavement support and coverage

Assist with bereavement services and provide coverage during staff absences, vacancies, increased workload, or other operational needs, as assigned.

Complete or support grief and bereavement risk assessments, individualized follow-up plans, condolence calls, family outreach, and timely documentation.

Provide grief support and counseling within professional scope and competency; assist with support groups, educational resources, memorial services, and community referrals.
Quality and operational responsibilities

Support compliance with applicable hospice requirements, accreditation standards, confidentiality obligations, professional standards, and agency policies.

Review records and service trends; address missed visits, overdue assessments, incomplete care plans, documentation gaps, and delayed bereavement follow-up.

Participate in quality assessment and performance improvement (QAPI), incident and complaint review, survey readiness, staff education, and corrective action follow-up.

Collaborate with hospice leadership on staffing, service capacity, resource use, and workflow improvements. Complete assigned reports and communicate barriers promptly.
