Salary: 75k-83k & up to $20k bonus

About the Position

The Community Partnership Manager has a passion for serving older adults through compassionate, high-quality care. This individual will leverage healthcare expertise and strong community relationships to expand awareness and access to our service lines throughout their assigned territory.
As a trusted partner to physicians, hospitals, and community-based organizations, this role is responsible for cultivating and sustaining a strong referral network while representing Goodwin Living Health’s full continuum of care. By staying attuned to market trends and community needs, the Community Partnership Manager will help ensure older adults and their families experience seamless, coordinated, and dignified care at every stage. This role plays a key role in strengthening Goodwin Living Health’s presence and reputation as a leading provider of home-based and end-of-life care in the region.


Job Duties

Healthcare & Community Partnerships

Serve as the primary Goodwin Living Health representative to physicians, hospitals, assisted living communities, skilled nursing facilities, and community partners within the territory.

Build trust-based, long-term relationships with care coordinators, physicians, social workers, discharge planners, administrators, and executive leadership.

Generate referrals and admissions for service lines by cultivating relationships with hospitals, SNFs, physicians, independent and assisted living facilities, and other community resources.

Educate partners on Goodwin Living Health's full continuum of services and how they support quality of life, safety, and dignity.

Assist with public relations outreach through speaking engagements, in-services, and active involvement in community organizations.

Continuum of Care Leadership

Promote integrated care across home health, hospice, home care, rehabilitation, and other service lines ensuring patients and families experience continuity rather than fragmented services.

Identify changes in patient condition, caregiver stress, or functional decline and facilitate timely transitions to appropriate levels of care, including hospice when indicated.

Support referral partners in navigating complex patient and family needs.

Growth & Market Development

Develop and execute a territory growth plan aligned with organizational goals for home health and hospice growth.

Grow and expand market accounts related to service lines; conduct market analyses and develop sales strategy and goals in collaboration with senior leaders.

Conduct marketing and sales calls; evaluate results and effectiveness of sales activity.

Establish and maintain a robust network of referral partners across medical, residential, and community settings.

Monitor the external and internal environment for development of new market segments and trends.

Support market research plans for services; collaborate on brand positioning, databases, and direct response strategies.

Track and follow up on referrals, opportunities, and relationship development activities.

Collaboration & Advocacy

Serve as a liaison between referral partners and Goodwin Living Health clinical and operational teams.

Advocate for patient and family needs, helping ensure seamless admissions, communication, and care transitions.

Participate in care conferences, case discussions, and community meetings as appropriate.

Support business development activities and help establish strong relationships with new and existing referral sources and accounts.

 

Job Requirements

Education & Experience

Minimum of a Bachelor’s degree in communications, business, marketing, or related field.

At least one year of recent marketing experience in home health or a similar industry, with at least three years of marketing experience in the healthcare industry.

Proven ability to develop and implement a sales and marketing plan with demonstrated achievement of referral goals within the market.

Excellent planning, organization, and presentation skills.

Outstanding customer service, verbal, and written communication skills.

Knowledge of and ability to use technology to access various markets and execute outreach strategies.

Proficient computer skills including documents, spreadsheets, and database management; ability to learn new electronic medical record software.

Able to work independently and thrive in a collaborative team environment.

Able to perform detailed work with a high degree of accuracy.

Able to maintain confidentiality of data.
