You will work with health homes, crisis, withdrawal managements and other teams, navigating through the health system. Connect clients with appropriate resources and services by understanding the client's needs and developing individualized case plans.

What we need for this job:
- Bachelor's Degree in Psychology, Social Work, Counseling, or related field preferred or one-year experience in the behavioral health field for non-related Bachelor’s degree
- Experience as a Case Manager
- Experience working with adults and/or children in an outpatient setting
- Must have current CO Driver’s License and a safe driving record
- Comfortable traveling to different locations
- Your work location may fluctuate between the office and out in the community based client need situations, 5 days a week
