Purpose:
The ACT Employment Case Manager works collaboratively as part of an Assertive Community Treatment (ACT) team to partner with clients, adults 18 yrs. and older with severe mental illness to promote recovery and support to their families and networks by providing assessment, planning, linkage, coaching, monitoring, supportive counseling, and advocacy services. You will work closely with your teammates in interchangeable roles in order to provide seamless continuity of care for clients when necessary. You will report to the ACT Manager.

What we need for this job:
- Bachelors in Psychology, Social Work, Counseling or related degree
- Collaborative, and client centered approach with 1 year of clinical experience
- Organization is essential to managing client information, appointments and follow up items
- Collaborative team member who is ready to jump in, support our clients and team
- Experienced community outreach with your ability to connect our clients with community resources
