Licensed Clinical Therapist/Social Worker

Job Description

PRINCIPAL RESPONSIBILITIES:

Under the direct supervision of the Clinic Director of St. Anthony Medical Clinic (SAMC), the Licensed Clinical Therapist/Social Worker will provide therapeutic mental health services to clients of the SAMC and other St. Anthony Foundation programs. The provider will function as part of the primary care team at the Clinic. The individual will have demonstrated skills and ability in promoting excellent patient care in a patient-centered medical home setting with integrated primary care and behavioral health, and will also act as a behavioral health resource and consultant to the health care team that includes medical providers, support staff, health educators and other St. Anthony programs.

MAJOR DUTIES:

  • Provide change-focused short term therapy, consultations and referrals for Clinic patients on a drop-in, appointed time and group visit basis.
  • Provide counseling services for individuals, families, and groups, as appropriate.
  • Conduct clinical assessments; develop treatment plans, and appropriate services and follow-up.
  • Refer clients for behavioral health and other services as appropriate.
  • Exchange information (within legal parameters) with other health and/or rehabilitative agencies to provide continuity of care for clients receiving services from more than one agency.
  • Facilitate case review meetings and participate in staff meetings, agency meetings and other meetings as assigned.
  • Serve as a crisis counselor for patients.
  • Work in an integrated setting offering collaborative care with primary care teams via consultation, shared patient visits, warm hand-offs or other forms of patient care integration activities.
  • Serve on committees and participate in various health center projects as assigned.
  • Assist in the design and implementation of behavioral health and collaborative care protocols.
  • Licensed LCSW or Licensed Psychologist to provide workflow review and training of the Behavior Heath content in the Electronic Health Records (EHR).
  • Dual signature on all charts; Periodic review and chart audit including Peer Review to ensure compliance of charting protocol.
  • Complete all necessary reporting of supervision hours to the corresponding board or licensure agency of the supervisee.  
  • Perform other duties as assigned.
Skills & Requirements

MINIMUM QUALIFICATIONS:

  • Licensure in the State of California as an MFT, LCSW or doctoral-level clinical psychologist.
  • Fluency in English and Spanish preferred. Bilingual in Cantonese and Vietnamese also helpful.
  • 3-5 years counseling or case management experience desired with proven expertise in assessing and managing the psychosocial needs of diverse, low-income and medically underserved populations. 
  • Ability to dedicate at least 3 hours per week of case management review with non-licensed providers.  
  • Ability to provide onsite consulting support to non-licensed provider.
  • Experience in the assessment and treatment of psychological trauma.
  • Knowledge of interrelationship between chronic illness and mental health including addictions.
  • Knowledge of and experience with crisis intervention theory, cognitive behavioral treatment, short-term treatment strategies and practice techniques.
  • Ability to integrate therapeutic mental health services with services provided by other disciplines, especially primary care.
  • Ability to assess, advocate for and provide interventions and treatment to clients in an individual, family or group setting.
  • Familiarity with psychotropic medications and experience working with patients on such medication.
  • Experience and skill in evaluating and treating children and adults with mental disorders and stress related to poverty, homelessness and immigration history.
  • Ability to work in a medical environment at the clinic level and effectively and comfortably work as a member of an interdisciplinary team including addiction treatment and a medication assisted treatment  Medication Assisted Treatment (MAT) program.
  • Demonstrated ability to exercise sound independent judgment and effectively manage time and resources.
  • Familiarity with or skills necessary to use electronic medical record software.
  • Knowledge of applicable mental health laws and ethical standards.
