Job Description

This in office position is a quality based, detail-oriented position is located in Tulsa, Oklahoma.

Office Hours: Monday through Friday from 8:30am to 5pm

This position coordinates all aspects of the patient care program, working with the Nursing team, Home Connect Coordinators, internal partner departments, referral sources, and patients to ensure seamless care. The role involves coordinating documentation and deliverables with treatment teams, reviewing and tracking nursing documentation, and managing medical records requests. Additionally, the position handles patient inquiries, conducts quality audits, and supports team processes. This is an in-office role.

EDUCATION AND EXPERIENCE:

  • Graduate of an accredited school of nursing required
  • Licensed Practical Nurse (LPN), in good standing with the practicing state’s Board of Nursing required
  • At least 1 year of acute care or specialized alternate site IV therapy experience preferred
  • Home Infusion experience preferred
  • Understanding of managed care organizations or insurance preferred.
  • Proficient computer skills un the use of Microsoft applications and Electronic Medical Records.

Essential Duties and Responsibilities:

  • Upholds patient rights, privacy, and confidentiality.
  • Collaborates with nurse leadership, supervisors, and field nurses to maintain the database and follow processes.
  • Works with the nursing team, including the Administrator and Governing Body, to ensure accurate patient care documentation and data collection.
  • Reviews and audits patient medical charts for accuracy and completeness.
  • Tracks provider compliance with required patient discharge documentation, verbal orders, and plans of care.
  • Manages patient inquiries related to Alpha 1 deficiency test kits and provides education on their use.
  • Participates in nursing multidisciplinary meetings and generates medical records for legal and compliance teams as needed.
  • Maintains patient records in compliance with HIPAA, state, and ACHC/URAC regulations.
  • Provides high-quality customer service to patients, providers, and internal teams.
  • Coordinates services and communication between customer service, financial services, pharmacy, and nursing departments.
  • Manages emails, voicemails, and faxes in a timely manner and supports the field nursing team and management.
  • Cross-trains in Home Connect Coordination, including intake of patient referrals, managing patient flow, data entry, and coordinating supplies and services.
  • Ensures all interactions are professional and ethical, complies with company policies, and completes paperwork on time.
  • Shares knowledge with team members and performs other job-related duties as assigned.
